Lawanda is a 47-year old female referred to us from Waccamaw Center for Mental Health for assignment to a medical home and to obtain assistance for her medications. During enrollment, it was noted that the patient had been using the ER department for her primary care and for help in controlling her diabetes and hypertension. She was enrolled in HOP and made an appointment with Little River Medical Center. Since her first appointment, the patient has been very compliant with her medical provider, her diabetic counselor, and her mental health counselor. She has been very disciplined and eager to learn about methods of managing her diabetes. Lawanda is enrolled in Welvista and received a glucometer and testing strips. Since becoming part of the AccessHealth Horry family, the patient has had no ER visits and is doing very well managing her health.

Samuel is a 58-year old Caucasian male who has been enrolled in the HOP program for a year. He states he is very proud of the success he has made in improving his health and the positive lifestyle changes he has made, thanks to the guidance and support received from AHH staff. When Samuel was enrolled in April 2019 at Grand Strand Regional Medical Center, he had just suffered a major heart attack and had cardiac surgery. He had been smoking a pack of cigarettes a day along with poor eating habits and consuming 4-10 beers daily. AccessHealth Horry was able to enroll him in the HOP program while he was still inpatient. He chose Little River Medical Center as his new medical home and was approved for Welvista before he discharged from the hospital. He has made all of his follow-up appointment, including his cardiologist appointments and has since graduated from cardiac rehab. AccessHealth Horry casemanagers and nurses have helped guide him in the direction of better lifestyle choices and a year later, he is 100% sober, has cut all fried foods from his diet, is walking 2-3 miles a day, has quit smoking, and has found a new job in construction. He states he is very happy to be healthy and back to work and grateful for the intervention of AccessHealth Horry in his life.

Virginia is a 50-year old female with diabetes, hypertension, and dyslipidemia. She has a history of transient ischemic attacks. She was referred from Conway Medical Center to AccessHealth Horry's HOP program. She chose Health Care Partners as her medical home. She was approved for the Welvista program and remains compliant with all her medications, lab draws, and medical appointments. Virginia's weight has decreased by almost 10 pounds and she continues to do well with assistance from AccessHealth Horry.

Jerome is a 47-year old white male who came to us with uncontrolled diabetes, cirrhosis of the liver, anxiety disorder, and alcohol addiction. He was referred by McLeod Seacoast in February 2019 and enrolled in the HOP program. Jerome chose a new medical home and received treatment from our partners at Waccamaw Center for Mental Health and Shoreline Behavioral Health Services. Since his referral, he has been successfully enrolled in Welvista for the past two years. The AHH staff referred him to a gastroenterologist and helped him obtain food stamps (SNAP). Jerome has been clean and sober since enrolling in the HOP program almost two years ago. He remains compliant with all his medications and his medical appointments.

Tonya is a 54-year old African American female with a diagnosis of congestive heart failure. She frequented a number of hospitals ERs over the years, rather than establish care with a primary physician. She did not take any blood pressure medications or monitor her blood pressure. When AccessHealth Horry met her at Grand Strand Regional Medical Center, her Casemanager and Welvista Advocate were able to have a frank discussion with her, emphasizing the importance of follow-up care, taking the proper medications regularly, and seeing a primary care physician. Once Tonya realized AccessHealth Horry was there to help her each step of the way and assist her with her medications, navigating the healthcare system did not seem so overwhelming anymore. Tonya went to her doctor's appointment as scheduled and was excited to be on the right track. She has been feeling better and is so grateful to have support and guidance. Now that Tonya does not have a barrier to affording her medications, she is more willing to participate and eager to see her doctor on a regular basis.

Jennifer is a 45-year old female with diabetes, hypertension, asthma, and arthritis; she is also bipolar. She has limited income and is unable to work, dependent upon her family for financial support. She entered the HOP program in January 2019 and during the past year, has become very compliant. Her A1C is controlled with oral medications supplied by Welvista. She goes to Waccamaw Mental Health every Monday and receives counseling once a month. Jennifer has a very complicated set of treatment needs but continues to improve since receiving help and becoming compliant with her treatment plan.

AccessHealth Horry met Edward while he was admitted in Grand Strand Regional Medical Center in December 2018. His blood sugars were extremely high and he had an infected diabetic ulcer in his toe. Edward had just moved to the beach from California, where he was homeless. He was unable to find any healthcare or diabetic assistance while in California and his health had deteriorated. AccessHealth Horry made it possible for Edward to not only receive regular care from a primary care physician but to also receive free insulin and medications from Welvista. His AHH Case Manager taught him how to monitor his blood sugars, the first time he had understood how to do so in 35 years! Edward has been compliant, seeing his physician regularly and monitoring his blood sugars every 3 hours. Prior to receiving assistance, Edward only checked his sugars when he felt unwell and administered insulin if he had it. When he stopped by to visit AccessHealth Horry staff in January, staff did not recognize him! He looked healthy, happy, and confident. Edward stated, "I have no more pain or symptoms and feel better than I have in a long time!"

Sally is a 56 year-old female referred to us by Conway Medical Center. She was diagnosed with diabetes, hypertension, COPD, and depression. An AccessHealth Horry Care Coordinator enrolled her into the HOP program and Welvista. At the time of enrollment, she had numerous blisters on her lower extremities, which were open and draining. She was referred to Friendship Medical Clinic as her new medical home. Her A1C numbers have dropped, she goes to her doctor as scheduled, and her multiple illnesses are under control, thanks to her Welvista medications!

Becky is a 56 year-old female with diabetes and hypertension. She had 3 ER admissions in the summer of 2015 prior to enrolling in HOP and Welvista through AccessHealth Horry. Her AHH Case Manager assisted her in finding a medical home and enrolling in Welvista. A home visit was made to check her blood sugar, blood pressure, and to discuss her diabetes and hypertension. Becky was provided with a glucometer, strips, and a donated blood pressure monitor to assist her in monitoring her chronic illnesses. She received education about the importance of diet and how to best manage her condition. Since being enrolled in HOP and receiving support from AHH staff, Becky has not been back to the Conway Medical Center ER.

Julie is a special 37 year-old HOP client who enrolled with AccessHealth Horry in July 2018. She left school early to work with her mother and was unable to finish school. She needed assistance understanding commonly used medical terms and had significant problems with her memory due to severe seizures. As soon as AccessHealth Horry established a medical home with a primary care physician, Julie's seizures became more controlled. Unfortunately, Julie was also diagnosed with several large cystic fibroids, which caused her a lot of pain. AccessHealth Horry was able to get Julie a specialty referral and a consultation with an OB/GYN. Because Julie had difficulty with her memory, it was hard for her to fully comprehend the problems she was experiencing along with the recommended surgery. Julie was very thankful to AccessHealth Horry. "It was such a huge relief to find out what was happening. No one has ever taken the time to help me understand what was happening or get me the help I needed!"

Mark is a 27 year-old male with diabetes 1, admitted to GSRMC with diabetic ketoacidosis. He has a history of brittle diabetes, thyroid issues, and hypertension. Mark habitually seeks his medical treatment for his diabetic care and treatment at GSRMC. The patient was in the hospital for three weeks and unable to afford his doctor's visits, diabetic supplies, or medications. Mark was enrolled into HOP and an AccessHealthHorry casemanager assisted him in locating a medical home. He was approved for Welvista for his medication assistance. Since Mark discharged home, he has been seen by his primary care provider four times. He has had zero visits to the GSRMC ER since enrolling as a HOP patient in July.

Tom was enrolled in the HOP program in May of 2017 after having a stroke at McLeod Loris Hospital. He was diagnosed with hypertension and asthma. Tom was assigned a medical home, enrolled in Welvista, and has been an excellent client. He has not had any further hospital admission or ER visits since enrolling with AccessHealth Horry. Staff maintains contact with Tom and he is very satisfied with the support and benefits he receives from AccessHealth Horry and partnering organizations.

Walter was admitted to a local hospital in April 2017 for congestive heart failure and hypertension. He was seen by an AccessHealth Horry "Care Navigator" who enrolled him into HOP and Welvista, helped him select a medical home, and made him an appointment to see the physician. Walter has been very compliant with his medications and the advice his physician gives him. He has started eating a heart-healthy diet, walking for exercise, going to his doctor appointments, and making sure he gets his medications refilled through Welvista before he runs out. Walter is an excellent example of how the HOP program can make a huge difference in an individual's life!

She was seen by her medical home provider and has not had to return to the ER since enrollment. She was referred to the Waccamaw Center for Mental Health and is doing very well. She is now compliant with her medications and follows her doctor's instructions.

Samantha was a 30 year-old client who was enrolled in the HOP program in January 2017. She was divorced, lost custody of her daughter, lost her job, assaulted her sister, and was prostituting for drugs. Samantha hit "rock bottom" and reached out to AccessHealth Horry for help. She has since received assistance from one of our behavioral health partners, is receiving medications from Welvista to manage her illnesses, and is seeing her new physician on a regular basis.
