Apply for Assistance

Our mission is to increase the quality of life for the uninsured patients of Horry county by finding the patient an affordable medical home and locating available resources for the patient where possible. Patient Navigators and Community Health Workers meet with patients and/or their caregivers while they are in the hospital or by telephone to:

  • Evaluate the patient’s condition(s) to see if he/she qualifies for our services
  • Assess patient's self-management skills and needs
  • Make necessary referrals for any assistance programs and resources for the patient (where possible)
  • Refer patient to a local medical home for management and follow-up of his/her chronic illness
