Healthy Outcomes Plan (HOP) Patient Care Navigator
AccessHealth Horry partners with Conway Medical Center, Grand Strand Regional Medical Center, and McLeod Loris Seacoast to help chronically ill patients enroll in our Healthy Outcomes Plan (HOP). Several years ago, legislature asked hospitals to devise “healthy outcome plans” to focus on the most frequent, uninsured users of their emergency rooms with chronic health problems such as diabetes, cardiovascular disease, hypertension, sickle cell, HIV and AIDS.

Patient Care Navigators work with hospital staff to identify patients who would benefit most from HOP services, providing case management services and expertise to patients at no additional cost. Navigators help patients find a "medical home" with a primary care physician, often at a reduced rate. Patient Care Navigators can also refer patients to other community resources, including mental and behavioral health providers.
